EU health card

When you travel to EU for conferences, workshops etc. you must do so with a valid EU health card. Núria V. *must* upload it to the intranet when communicating your trips to Human Resources (HR).

If you're a EU citizen you can request the card online and it’ll be sent to your home, it’s valid for 2 years.

If you're from a non-EU country then you need to ask for an appointment at the "Seguridad Social" website and go in person to get a certificate that is only valid for 1 trip. Warning: You’ll be asked for your Social Security Number “Nº de la Seguridad Social”, you’ll find it on your contract but make sure you key in yours and not ICIQ’s as both are on the contract! Look for “Nº del trabajador” or “Nº del afiliado”

HR still have to let the Social Security know when you’re travelling, so Núria V. needs your docs and to fill in the form on the intranet.
